Images from hip MR arthrogram show a partial tear at the foveal insertion of the ligamentum teres with diffuse ligament thickening, in addition to the obligatory anterior labral tear. She experienced no pain relief with intra-articular lidocaine (not sure what that means either way). I know from personal experience that ligamentum teres tears can be very symptomatic, whereas, in some, labral tears can be relatively asymptomatic in patients with an occasional sensation of instability and clicking, but without significant pain. In all likelihood, however, with a labral tear and no degenerative cartilage loss (and health insurance), it is the labral tear that will be addressed arthroscopically. Given time and rest (and not active PT) I think the ligamentum teres tear will likely heal. [ Article ]
