The DPM referred him with Hallux IPJ sesamoid and bursitis. There is a liquefactive calcific deposit plantar medial to the IPJ capsule with milk of calcium fluid fluid level. The adjacent capsule is frayed and bright; this presumably arose from the capsule. I had her send me screenshots of his XRs, you can see why she mistook it for a sesamoid, but sesamoids should not grow. There is a normal sesamoid embedded in the IPJ plantar plate in slide 3.
