3 weeks post op he had dislocation of the polyehtylene cup, and had a poly exchange. 3 days after that, he felt recurrent discomfort. Now 3-4 weeks later, AP and Y view shoulder XRs suggest malalignment of the glenosphere and humeral articular surface, but CT is confusing. Despite IMAR there is limiting streak artifact. Glenohumeral components seem to articulate. In all 3 planes of CT there is a dislocated lucent polyethylene cup in the subacromial space. In consultation with the surgeon, as I confessed that I didnt have a clue how to explain the mismatch between the XR and CT appearance, we agreed that an Axillary XR might be helpful. Only the Ax view shows dislocation of the glenosphere, which is disarticulated from the metaglene. Both the metaglene and humeral stem appear solidly in the glenoid and humeral shaft.
