Pre-op images slide 1 show normal bone marrow signal and degenerative intrasubstance signal in the PHMM which questionably extends to the joint surface (not reported definitively as tear). 2 months post-partial meniscectomy, the central portion of the PHMM is subluxed into the posteromedial intercondylar notch, the peripheral remnant is extruded and there is a small fracture (not subchondral) in the MFC with surrounding edema. 5 months post-op it looks like extensive subarticular AVN (double line sign, new flattening of the subchondral cortex). Is this all a function of increased contact pressure following meniscectomy leading to fracture, which progresses to AVN? Is this an unavoidable small post-op risk, or is this related in some way to the surgical technique? References are vague at best.
