There is normal variant anatomy of the PTT that is not described among the reported normal variants (see ref). There is no insertion onto the navicular. The major slip is tendinotic and partially torn (white arrow), coursing plantar to the navicular to insert on the plantar base of the medial cuneiform. Note that the normal Ant tibial tendon inserts more distally on the plantar medial head of the medial cuneiform (slide 2). There is a smaller accessory slip of the PTT inserting onto the plantar base of the lateral cuneiform (red arrows). Just crazy. [ Article ]
