Peroneus longus tears at the midfoot are relatively uncommon and are often attributed to friction with a large lateral calcaneal peroneal tubercle, or can be associated with an os peroneus. This patient has neither. I am surprised that the thickened retracted proximal remnant of the peroneus longus was not appreciated by the examining orthopedist on ultrasound.
