10% of tibialis anterior tendons have variant distal insertions, the most common of which is a bifid insertion onto the 1st MT base and medial aspect of the medial cuneiform. As we see here. In this woman, the 1st MT insertion is normal, but she has tendinosis with peritendinous edema of the more proximal superficial portion inserting onto the medial cuneiform. In these slides, the red arrow points to the normal portion inserting onto the 1st MT, The curved arrow points to the tendinotic portion inserting onto the med cuneiform. I imagine it might be easy to mistakenly call this variant a split type tear; dont do that. [ Article ]
