lateral talar process fracture
This is the same man with the accessory FHL. Although there is moderate marrow edema related to the impacted, expanded lateral talar process, I think this is likely an old intra-articular fracture, with marrow edema related to post traumatic degenerative change, partial fracture non-union (arrow points to small defect in sag image) and lateral impingement surrounding the bony prominence. Up to approx 40% of these fractures, which are common among snowboarders, are missed. There is medial downsloping of the middle subtalar joint with mild prominence of the middle talar facet (slide 4); this does not look typical of non-osseous coalition. I am wondering if this suggests remodeling due to the lateral process fracture which might have occurred prior to skeletal maturity. By the way, the ATFL and CFL are completely torn. [ Article ]
