There is an acute on chronic high grade partial tear at the origin of the proximal hamstring tendon complex. A mature traction spur at the lateral margin of the ischial tuberosity narrows the ischiofemoral space (9.4mm) and results in ischiofemoral impingement. There is a partial instrasubstance tear and surrounding edema of the quadratus femoris. Note min bright signal in overlying sciatic nerve. Symptoms related to proximal hamstring tendon pathology and ischiofemoral impingement can mimic one another, in this case, the patient has both. [ Article ]
