Discussion

Subcortical fracture in a 67 y/o female is most likely insufficiency (versus stress fracture). These are associated with root attachemnt tears of the medial meniscus. Note the image with the arrow showing the ghost meniscus sign of a radial root attachment tear of the posterior horn of the meniscus. Unstable meniscus with slight peripheral subluxation predisposes to this type of fracture.
