Discussion

History is suggestive of infection. Involvement of the C-C ligament is unusual in a typical infection that would also have to involve the AC joint and early findings of synovitis of the shoulder. Turns out that he was just hospitalized for acute gout a month before and has been on colchicine and steroids. He abruptly stopped the steroids and went into Addisonian crises. Was hospitalized for pain and an increased WBC count and was complaining of shoulder pain and had a new doctor taking care of him. Hence the MRI of the shoulder to r/o abscess. Reference article.
