Discussion

There is medial dislocation of the PTT which is mildly thickened but intact, with delaminating tear of the flexor retinaculum. The intact retinaculum stabilizes the PTT posterior to the distal tibia and medial malleolus. There are surprisingly few reports of these injuries. Prior MRI 11 mos earlier, 1 month after work injury at that time shows the PTT was located and the flexor retinaculum was intact.
