History

50M with left shoulder pain x 3 days. Minor trauma 1-2 mos ago with vague persistent symptoms.

Discussion

Fell 2 months earlier with minor persistent symptoms, acutely worse x 3 days opening a door. He reported night pain and pain with reaching. 80% improved after 2 days on Medrol dose pak. MRI shows subcortical humeral shaft marrow and parosteal edema with dark signal foci in the region of the pectoralis major insertion. CT shows both peri-tendinous calcifications as well has HADD deposits eroding through the cortex and into bone. Axillary XR clearly shows the peri-tendinous deposit; the cortical erosion is faintly visible....and I expect you can all see the intra-osseous calcific deposit.
