Discussion

There are massive cuff tears, sparing only the teres minor.
Check out the course of the torn subscapularis tendon. It is displaced into the posterior GH joint. The LHB tendon is intact, but is dislocated into the shoulder joint taking a crazy course around the posterior aspect of the humeral head on its way to the glenoid rim.

Diagnosis

tendon of long head of biceps brachii ( RID1944 )
