Discussion

This is an incidental finding. I have never seen an accessory FHL, nor can I find a reference. This is not the relatively common FDAL, as it lies deep to the PT neurovascular bundle. In the 2nd slide you can see the tendon of the more medially located accessory FHL blend with the proper FHL tendon at the level of the subtalar joint. I include the reference of the commonly recognized accessory muscles of the ankle. This is not mentioned. Reference article.
