45 yr old male with 6 months bilateral shoulder pain and LROM

Discussion

Despite the female predilection for adhesive capsulitis, here is a case of bilateral adhesive capsulitis in a male, more severe on the left. Up to 40-50% sequential bilateral involvement has been reported, though simultaneous bilaterality is rare. In the left shoulder, bright signal at the articular surface of the cuff near the supraspinatus insertion is more likely capsular inflammation than cuff pathology (not seen on the Rt where involvement is more limited to the axillary recess). Note the disproportionate fluid distention of the biceps sheath, more apparent on the left.
